As the number of systemic psoriasis therapies increases for the treatment of moderate to severe psoriasis, a shift from traditional agents (i.e., methotrexate, cyclosporine, retinoids, phototherapy) to biologic agents is anticipated. The introduction of these agents has changed the treatment of psoriasis in patient population by reducing the use of traditional systemic agents: new therapies might improve the overall quality of life with awareness and attention to comorbidities but the process of translating psoriasis guidelines into practice reveals important gaps meaning that they lead to changes in care that benefit not so many patients. This review provides an insight into various old and emerging therapies along with the real-life trends and impact on currently approved treatments for psoriasis.
